How to Register
If you live in our practice area and would like to register with us simply come into the surgery where one of our receptionists will make an appointment with either the Practice Nurse or our Healthcare Assistant to complete a new patient health check. You will be given forms that need to be filled in prior to your appointment. Alternatively you can download and complete the forms shown below.
If you live outside of our practice catchment area (Gateshead) you are welcome to register with us, however we would not be able to visit you at home in the event that you were unable to get to surgery. Contact reception for more information.

Download a form to opt out of having a Summary Care Record (SCR) or opt out of additional information being included in the SCR.
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
