Teresa is responsible for all aspects of managing the practice and developing and improving the services we provide.
Mel Shotton – Operations Manager
Mel is responsible for the operational management of the practice. If you have any feedback you would like to give about the practice, Mel would love to hear.
Patient Care Navigators
Our patient care navigators are your first point of contact within the practice. They carry out a wide range of administration tasks. They have completed training in Active Signposting to enable them to “navigate” you to the right person or service. This ensures that you get the right care with the right person at the right time. To enable them to do this they may ask you for some information about the problem you have when you contact the practice. Our Patient Care Navigators are bound by the same rules around confidentiality as our GP’s.
Michelle Fraser, Andres Chiraboga Guevera & Zach Hardy are the Care Navigators hear at Second Street Surgery. If you or a relative registered with the practice require support with social problems (for example debt, isolation and loneliness, healthy living or housing issues) They can help by signposting you to services that can help and ensuring that you get the support you need.
